To complete an Agency Registration Request, you will need the following:
- Name of agency
- Agency type (based on agencies listed in the statute)
- Contact information, including address, email, and phone
- Agency license number (from the Colorado Department of Public Health and Environment)
- Vendor name and contact information (if your agency uses a vendor to complete background checks)
*Due to the COVID-19 pandemic, please submit all requests via the online tool.
If you do not have access to submit an online request, you may submit the paper form. Please note that this will result in long processing delays.
